A 30 years, old guy, otherwise apparently healthy without any previous health issues developed sudden shortness of breath and left sided chest pain with giddiness, lasting for few hours. Chest pain was so severe that he was feeling suffocating progressively. He was working in a financial company and used to do accounting work. He never had any addiction in life, nor was he taking outside junk foods. No body from the family had any heart related ailments. He assumed his symptoms as an acidity initially and took over the counter medications for same which didn't give any relief to him. When it was worse he rang up his neighbor, who shifted him to Hospital in the late evening hours. We found him in a collapsed state with pounding heart beats, shortness of breath, low blood pressure. As he had desaturation, he was given oxygen support and his initial blood investigation workup was done. His ECG revealed no significant change except a tachycardia. He was slightly overweight. History revealed, stressful life, prolong sitting hours for job purpose with no regular exercise. Pulmonary embolism was suspected. (Possibility of clot in the major blood vessel of lungs- pulmonary artery). There were no signs of Deep Venous Thrombosis. Immediately echocardiography was done which showed dilated right sided chambers with elevated pulmonary artery pressures which was indicating towards pulmonary embolism. He was given loading anti-platelets and heparin with inotropic support. Urgent CT pulmonary angiogram was done which showed thrombus in right pulmonary artery occluding it. Immediate decision was taken and he was given, Alteplase, thrombolytic agent to lyse the clot. He showed marked improvement in overnight ICU stay. Next day he was asymptomatic, with normalization of pulmonary pressures on ECHO. He was put on oral anticoagulation. He was found to have high cholesterol, deficiency of anticoagulant proteins, which was addressed and discharged on 5th day. Next 6 months he was on regular follow up and sincerely followed doctor's orders regarding medications and lifestyle modification. This event changed his life in a positive way where he started taking care of himself. He started regular brisk walking (30-45 minutes daily), strict low cholesterol, low carbohydrate, high fiber diet, and meditation. His routine was normalized within in a month span. He followed up his deranged biochemical parameters which improved to normal in 6 months. Repeat CT chest was normal. He brought his weight to normal in 6 months. He knows that less physical activity, sedentary life style, faulty dietary habits threatened his life and was lucky to get through it. It's new beginning for him and wants to continue healthy life in new spirits. Considering present stressful life, we are neglecting our health by having some excuse which makes us prone for such scenarios. One should not neglect any type of chest pain or shortness of breath as can be lethal. We should understand the risk factors by doing screening tests regular health checks, start regular exercise and modify diet to have healthy life. We don't know whether we will get Second chance. So start now. Regularly moving out for exercise, eating healthy, coping up with the stress of life and making others aware of your healthy experiences will empower our healthy life.
